FIRST VISIT

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. When you arrive, please be sure to check in with the reception staff and provide them with your completed patient information form and medical questionnaire (located here). Please view the COVID-19 tab for more information on additional safety precautions our office has implemented.

Please assist us by providing the following information at the time of your consultation:

• Your surgical referral slip and any x-rays if applicable.

• A list of medications you are presently taking and the names and contact information of your doctors.

• Any insurance information to expedite your reimbursement, if applicable.

• Print and complete the two forms located here. Please bring these to the reception staff when you arrive for your first appointment.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation and/or treatment visit
